Dear Parent or Guardian of 11th Grade Student:

All students entering 12th grade are required to have a Meningococcal (MCV4) booster vaccine if their first dose was administered prior to 16 years of age.  Students who had their first dose of the vaccine at 16 years of age or older will not be required to have another vaccine.

Oakville High School will host a Meningococcal vaccine clinic at school on Wednesday, April 5th and Thursday, April 6th during the school day.  Walgreens pharmacists will be administering the vaccine.  If your insurance is accepted at Walgreens pharmacies, Walgreens will bill your insurance company with a copy of your child’s insurance/pharmacy card.  No copay is required at the time of service.  Walgreens is offering the vaccine for $133.99 if uninsured or if insurance doesn’t cover Walgreens pharmacy.  This needs to be paid by check made out to Walgreens at time of service.  No cash, credit or debit cards will be accepted.

The student must have the following with them on April 5th or April 6th:

  •  Walgreens “Vaccine Administration Record (VAR)-Informed Consent for Vaccination” form completed and signed by parent.
  •  Copy of insurance card.

The VAR form will be sent home with students on April 3rd and is also available on our website under “Nurse News.”
